Hello everyone,

The CHAT team has decided to make a (much easier) change to the referral process.

Now, any professional can make a direct referral online by following these simple steps:

1) Click the link: https://elevatehealth.org/index.php/our-solutions/care-continuum-network/
It should take you directly to the Care Continuum Network web page.

2) Scroll down and click on “Sign up.”
Located under the words “Empowering Coordinated Whole-Person Care.”

3) Fill out the patient information as much as you can.
You can leave information blank or type in “homeless” if they don’t have an address.  
*if you would like to refer them to the CHAT program, please be sure to click that box.
*if you are unsure what program to refer the patient to, please check the box “Unsure.”

4) Please be sure to click “Submit” at the bottom of the webpage after you have filled out the patient information.

You are still able to call the phone number listed on the back of the CHAT pamphlet (253-331-2380) to refer patients to the program. I have attached that pamphlet in this email, as well as my contact information. Please feel free to reach out if any further questions should arise.
