I am a nurse and UW Doctor of Nursing Practice student who is doing a survey intended for service providers who help families with young children connect with socials services in Pierce County. I am working with the health department, 211, and Pierce
County Early Childhood Network. If this is work you do, your input on this survey would be invaluable so that we may better understand the strengths and challenges of family service linking, and what recommendations we can make for improvement in our current
linkage systems based on your feedback.

Dear Service Provider of Pierce County,
You are invited to participate in the
Parent
Child Health Service Linkage Survey. This survey aims to better understand the strengths and challenges related to successfully connecting families with young children (prenatal-3 years) where a parent has complex needs to the services they desire
in Pierce County. Questions ask about what you do to link these families to their desired services and what recommendations you have for improving the systems available in Pierce County. This survey should take about
5-10 minutes of your time and will close at 11:59 pm on
February 3, 2024.
About the Survey
Tacoma-Pierce County Health Department, Pierce County Early Childhood Network, and United Way of Pierce County are interested in understanding the strengths and challenges associated with helping young families with complex needs get connected to desired services.
This survey is intended for community health workers, social workers, outreach providers, and any service provider that works to connect families to needed and desired services. Your anonymous input will inform our recommendations for system improvements in
parent child health resource connection and will be shared with decision makers in Pierce County. A University of Washington Doctor of Nursing Practice student is supporting data collection and analysis.
